A completed, unsigned passport application (Form DS-11) filled out in black ink or completed online and printed single-sided. Form DS-11 and all other passport applications are available on the U.S. Department of State website, or at the Library.
Proof of U.S. Citizenship (one of the following must be submitted with application):
Proof of Identity (one of the following must be submitted with application):
A Passport photo (photo service available at the library for a fee of $15)
Please note: there are special requirements for children under age 16 and minors age 16-17. Refer to the U.S. Department of State website for full information on applying for a passport.

Renewal forms are available online through the U.S. Department of State website, or at the Library. The Library will not process renewal applications nor review documentation.
